2025 CHIP Update Meeting
Join Healthy Sheboygan County for an update on how we’re making a difference in the community through our Community Health Improvement Plan (CHIP).

A CHIP is developed based on the priority issues identified in a Community Health Assessment (CHA). The CHIP then supports active engagement by community members and community organizations in improving the health of Sheboygan County. Healthy Sheboygan County’s current CHIP cycle will run from 2024 through the end of 2028. 

Five priority areas were chosen for this cycle based on information gathered in the CHA: 
  • Positive mental health
  • Responsible substance use
  • Activity and nutrition
  • Access to affordable housing
  • Access to affordable, high quality child care
Each of these priority areas has a dedicated action team that meets regularly to complete the work outlined in the CHIP.

Current Grant Funding Opportunities
Maternal Health Innovation Evaluation Grant
Due September 22, 2025

The Wisconsin Department of Health Services (DHS), Division of Public Health (DPH) announces the availability of funds for the Maternal Health Innovation Evaluation grant.

This funding will support the development and execution of an evaluation plan for the Maternal Health Innovation Task Force and the Maternal Health Innovation grantees. The evaluation will utilize available statewide and grantee-level data to identify and analyze yearly maternal health measures to demonstrate impact.

The Maternal Health Innovation Program has access to statewide maternal health data from data systems, including vital records, hospital discharge records, the Wisconsin Pregnancy Risk Assessment Monitoring System, and the Wisconsin Maternal Mortality Review program.

The Maternal Health Innovation Program has also been collecting performance data from Maternal Health Innovation grantees to capture their activities and populations served.
Additional measures and data collection mechanisms may be newly developed as a part of the evaluation. Measures are expected to be qualitative and quantitative.

Reproductive Health and Family Planning Grants
Due September 26, 2025

The purpose of the Wisconsin Department of Health Services (DHS) Reproductive Health and Family Planning (RHFP) Program is to support a network of clinics across the state that provide client-centered quality RHFP services. 

The RHFP program coordinates care and focuses on comprehensive services by bringing together a variety of providers and referral partners for providing standard information, training, and resources to all partners. 

To aid in the provision of RHFP services, the RHFP Program announces these grant funding opportunities (GFO): 
  • Training for Colposcopic Examinations and Activities
  • Grant for Colposcopies and Other Services
  • Women’s Health Block Grant – City-based Clinics
  • Women’s Health Block Grant – Racial Minority
  • Women’s Health Block Grant – Chlamydia Outreach
  • Women’s Health Block Grant – General
  • Outreach to Low-Income Pregnant Women

There is Hope Walk: There is Hope Walk: Saturday September 13th at Deland Park in Sheboygan from 8:30am to 12pm.
Walk to End Alzheimer's: Saturday, September 20th at Evergreen Park in Sheboygan. The opening ceremony will begin at 9:40am and the walk starts at 10am.
Caregiver Conference: Please join us for the 2025 Sheboygan County Caregiver Conference to learn and stay updated with all the caregiving techniques, tools, and the resources you need for you and your loved one. Trunk or Treat: To celebrate National Weatherization Day and provide valuable community resources, Partners is excited to host our 3rd Annual Trunk or Treat & Resource Fair on Saturday, October 25. Join us along with local community agencies and partners as we will fill the lot for an afternoon of fun, candy, games, and resources for this free community event. Want to wear a costume? WONDERFUL! Costumes aren't required but they are encouraged and welcomed.
