The CHA/CHIP Cycle
  • Community Health Assessment is a report that looks at information from many sources to tell the story of health and well-being in Sheboygan County
  • Aims to increase community awareness and help community partners better understand where and how to focus resources to reduce health disparities and increase the health of all people
  • Community Health Improvement Plan (CHIP) supports active engagement by community members and community organizations in improving the health of Sheboygan County
  • Five priority areas chosen for this cycle: 
    • Positive mental health
    • Responsible substance use
    • Activity and nutrition
    • Access to affordable housing
    • Access to affordable, high quality child care

Community Health Assessment Steering Committee determines top health priorities

Partner Initiative Survey sent out to community organizations to determine what work is already being done in Sheboygan County

March 2024

  • 2023 Sheboygan County Community Health Assessment (CHA) published
  • Healthy Sheboygan County Call to Action meeting held to bring the community together for conversations around the health priorities from the CHA

April - June 2024

3 Action Team meetings held
  • Division of Public Health staff, partner organizations, and community members could work together to determine the goals, objectives, and strategies for each action team for the current CHIP cycle
  • Each monthly action team meeting built off of the one that came before it using logic models and guided conversations to determine the priorities within each team
  • Action Teams will meet regularly throughout the CHIP cycle to complete the work outlined in the CHIP

The Division of Care and Treatment Services is seeking applications for the development and operation of regional opioid and methamphetamine treatment centers to provide comprehensive substance use treatment services for people that present with a primary, secondary, or tertiary diagnosis of opioid use disorder or methamphetamine use disorder.

Law Enforcement Virtual Behavioral Health Program
Due July 26, 2024

The Division of Care and Treatment Services invites county agencies with a Wis. Admin. Code ch. DHS 34 certified crisis program to apply for funding allocated under 2023 Wisconsin Act 219 that aims to strengthen the collaboration between law enforcement agencies and county crisis services through the delivery of virtual behavioral health crisis services.

Positive Youth Development Programs
Due July 26, 2024

This funding will support local programs that aim to provide delinquent youth or youth at-risk of entering the juvenile justice system with essential resources. Programs must use a positive youth development framework for their programs.

The Division of Care and Treatment Services is seeking applications for a consumer and family education program that will support and create opportunities for consumers and families to be actively engaged in efforts to transform Wisconsin's mental health system in partnership with state and local agencies.

The Division of Care and Treatment Services is seeking applications to increase the accessibility of culturally competent substance use prevention and treatment services for the Deaf and hard-of-hearing community through outreach and training activities.

Urban Black and Hispanic Substance Use Disorder Treatment
Due August 15, 2024

The Division of Care and Treatment Services is seeking applications from public and nonprofit organizations to provide in-home and community-based substance use disorder treatment services directed at low-income Black and Hispanic individuals in urban areas of Wisconsin.

Statewide Crisis Intervention Program (SCIP)
County- & Tribe-Led Response & Harm Reduction
Due November 1, 2024

This SCIP funding aims to provide funding to counties and tribes across Wisconsin to support crisis response programs, suicide awareness, firearm safety awareness, trainings, and programming to decrease the amount of firearm deaths locally. Each county and tribe can implement programming specific to their local area and its needs to maximize impact on the community.
