Activity and Nutrition Produce for Pantries is a convenient way to donate fresh fruits and vegetables to help support individuals and families in need throughout Sheboygan County during the growing season. Purchase extra produce when you visit the SCIO Farmers Market at Fountain Park in Sheboygan this summer and place your donations in the collection box located at the SCIO’s information tent. Summer market hours are 8 AM to 1 PM on Wednesdays and Saturdays from June 3 through October 31.
Responsible Substance Use 394.75 lbs of unwanted medications were turned in across Sheboygan County on April’s Drug Take Back Day. Wisconsin leads the nation in collection for the 10th straight time. Drug Take Back Day events encourage the public to remove unneeded medications from their homes as a measure of preventing medication misuse.

2026 Community Health Needs Assessment (CHNA) Update |
We want to extend a heartfelt THANK YOU to everyone in the community who participated in our data collection for our Community Health Needs Assessment. We were able to hold 7 focus groups and had 1,541 responses to our online survey. This data is currently being analyzed and reports will be available for the community in early September. |
Sheboygan County YMCA Diabetes program |
An estimated 88 million American adults have prediabetes, and more than 8 in 10 have no idea they are at risk. The good news? You can reduce or eliminate your risk for type 2 diabetes by making small, measurable lifestyle changes.
The Sheboygan County YMCA’s Diabetes Prevention Program is here to give you the skills you need and the support you deserve. This CDC-approved, interactive course can be taken in-person or virtually, and features a supportive group environment to keep you motivated.
Program Highlights: Programs like this reduce new cases of type 2 diabetes by 58% overall (and 71% for those over 60). Participants receive a 6-month membership to the Sheboygan and Sheboygan Falls YMCAs (YMCA membership is not required to join the program). The program may be covered by your health insurance, and financial assistance is available for those who qualify.
Do You Qualify? You may be eligible if you are 18 or older, overweight (BMI ≥ 25), and at high risk for developing type 2 diabetes or have been diagnosed with prediabetes.

Opioid Abatement for Law Enforcement Agencies Due July 10, 2026
The Division of Care and Treatment Services is seeking applications to sustain law enforcement agencies providing medications for opioid use disorder (MOUD) in jails and educating staff on MOUD.
Funding is available for two activities. MOUD for people incarcerated with opioid use disorder: This funding is for law enforcement agencies with an MOUD program that provides all three forms of MOUD, including methadone, buprenorphine, and naltrexone, to people in jails. MOUD education: This funding is for law enforcement agencies with a MOUD program. It can cover the registration and travel costs for staff to attend a MOUD-focused training and/or costs related to overtime and backfilling to cover the post of a staff person away for a training.

Investing in Dental Care Technology: Grants to Improve Rural Dental Efficiency and Access Due July 27, 2026
The Wisconsin Department of Health Services (DHS) is seeking applications for competitive grants to support dental clinics in adopting technologies that improve efficiency and increase access to care in rural and semi-rural communities. Dental practices, particularly in more rural areas of the state, face challenges that result in reduced access to care. These challenges include high rates of uninsured patients, low and unpredictable patient volumes, dental workforce shortages, and limited capital resources to adopt innovative technologies. This funding opportunity seeks to address these challenges through increased access to dental technological innovations, which improve dental services in rural Wisconsin by increasing the speed of appointments while maintaining quality of care. This funding will allow dental practices to purchase technologies to deliver high-quality and efficient services. The Wisconsin Department of Health Services (DHS) seeks to strengthen the Wisconsin rural health workforce by developing and expanding high-quality and impactful community health worker (CHW) programs in clinical and community settings. Coordinating Care Across Wisconsin: Innovating Healthcare Through Partnership Grants Phase 1 Application due August 21, 2026
This five-year grant funding opportunity is intended to catalyze innovative healthcare service models through partnerships across communities, systems, and sectors in rural Wisconsin. Through these grants, the Wisconsin Department of Health Services (DHS) aims to address those points in a care continuum where community members are most likely to experience barriers to health-related services or fall out of care, and to directly address fragmented systems of care. Applicants may apply to multiple Rural Health Transformation Program (RHTP) funding opportunities for which their organizations are eligible.
Applications for funding will be released in two distinct phases: Phase 1: Planning grant. Applicants will submit a letter of application for an initial six-month planning or pilot grant. The planning period will provide an opportunity for awardees to receive technical assistance for application development, build a strong foundation for program implementation, and develop strong partnership models. We recognize that programs may be at different stages of development, from initial idea generation to ready-to-go projects and partnerships. We welcome a range of planning period activities. Phase 2: Full award. Planning grant awardees will be eligible to submit an application for a full award covering the remaining four-year grant period. The full application will be released in February 2027. Only recipients of an approved letter of application will be eligible to apply for the full award. Full award applications should reflect and build on the work done during the planning period and provide more in-depth program designs.
