Action Team Updates
The Sheboygan County Activity and Nutrition Coalition has been working together this summer with the SCIO Farmers Market and the Sheboygan County Food Bank collecting fresh produce for the Produce for Pantries program.  A total of 2,643 lbs of fresh, local produce was donated during the 2024 market season by the SCIO Farmer's market and distributed to Sheboygan County residents through the Sheboygan County Food Bank.
The Sheboygan County Housing Coalition hosted several events in recognition of Homelessness Awareness Month throughout November. One of these events was A Night Without Shelter, an event to build a deeper understanding of the challenges associated with homelessness. The coalition also collected winter wear and blankets at 3 Sheeps Brewing Company and Uptown Social throughout the month. These donations were given to Sheboygan Area Pay It Forward to be distributed to the unsheltered population in the coming months.
Community Health Improvement Plan
Healthy Sheboygan County has published the 2024-2028 Community Health Improvement Plan (CHIP)! The purpose of the CHIP is to improve the health and quality of life in our community. A big thank you to everyone who helped get this work done. Click on the image below to read the CHIP:

Current Grant Funding Opportunities
Funding Available to Increase Tenant and Homeowner Environmental Health Literacy
Due December 13, 2024

The Wisconsin Site Evaluation Program at the Wisconsin Department of Health Services is offering mini-grants of up to $15,000 each to help community-based organizations (CBOs) implement new, innovative, expanded, or enhanced interventions to increase tenant and homeowner environmental health literacy. We are seeking applications from CBOs interested in addressing environmental health hazards and their impact on housing. Health equity and environmental justice are priorities, and should be at the center of the project planning and implementation.

Maternal Medicaid Enrollment Projects
Due December 30, 2024

The Wisconsin Department of Health Services (DHS), Division of Public Health (DPH) announces the availability of funds to establish and execute Maternal Medicaid Enrollment Projects. This funding will support existing statewide or local organizations to specifically target the pregnant and postpartum (within ninety days) population for to support Medicaid renewals and applications. The purpose of this program is to reduce maternal mortality and severe maternal morbidity by supporting programming that improves maternal health and addresses maternal health disparities through quality services, a skilled workforce, enhanced data quality and capacity, and innovative programming.

DHS Epilepsy Services Grant
Due January 1, 2025

The Wisconsin Department of Health Services (DHS) distributes grant funds in accordance with Wis. Stat. § 46.57. Private non-profit organizations or county departments are eligible to apply for the Epilepsy Services Grant to provide direct or indirect services to or on behalf of persons with epilepsy or their families (or both).

DHS may not allocate more than $50,000 per year to any agency under this grant pursuant to Wis. Stat. § 46.57. DHS cannot award more than $150,000 total to all grantees per grant year.

Addiction Recovery Helpline

The Division of Care and Treatment Services is seeking applications for a phone line to provide Wisconsinites service referrals and resources related to substance use issues.
