The Sheboygan County Access & Navigation Certificate Program was nominated for the Working Together Award by the Sheboygan County Chamber. The team was recognized at the Chamber’s Annual Chamber Champions Gala on February 20, 2025.
Please consider joining the Access and Navigation Certificate program. This program is designed to educate Health and Human Services professionals, agencies, and other engaged individuals in Sheboygan County on how to build a bridge to help, connecting community members to vital county resources that ensure they thrive. This dynamic online course will begin on March 10 and end with an in-person community gathering on May 9.

The Sheboygan County Housing Coalition Education Committee presented to the City of Sheboygan's Committee of the Whole on the current state of homelessness within the city and county on February 24, 2025. The Advocacy Committee launched the first Renters' Association Meeting in February. Meetings will be held on the third Wednesday of the month from 5:30PM - 6:30PM at St. Luke's United Methodist Church (623 Ontario Ave., Sheboygan WI 53081).

Improving Heart Health Through Community-Clinical Linkages Due April 7, 2025
Wisconsin Department of Health Services Chronic Disease Prevention Program (CDPP) is seeking a community organization (e.g., non-profit, local, or Tribal health department, membership entity, coalition) who has a history of successfully working with clinical organizations (e.g., health system, federally qualified health center, free and charitable clinic, health plan) and other multi-sector and interdisciplinary partnerships to advance health and social outcomes in the community.
The applicant will lead an interdisciplinary heart health learning collaborative aimed at addressing the clinical and non-clinical barriers that have led to cardiovascular disease and poor health outcomes in our communities.
Due to existing funding commitments for heart health collaboratives in Milwaukee and Brown counties, the CDPP is unable to consider additional applicants from these regions.

Improving Access to Healthy Food in Communities with High Rates of Diabetes Due April 7, 2025
Wisconsin Department of Health Services Chronic Disease Prevention Program is seeking to partner with an organization to focus on implementing sustainable policy, systems, and environmental change efforts to reduce health disparities related to access to healthy foods in communities across Wisconsin.
The work will support community organizations and agencies who serve priority populations throughout Wisconsin.

Increasing Access to, and Participation in, Complementary Diabetes Support Programs Due April 7, 2025
Wisconsin Department of Health Services Chronic Disease Prevention Program (CDPP) is seeking to partner with an organization which holds an umbrella license to all Self-Management Resource Center (SMRC) programs, including the Diabetes Self-Management Program (DSMP) known as Healthy Living with Diabetes or Vivir Saludable con Diabetes, and provides training, technical assistance, and support to current or aspiring SMRC DSMPs.

Emotional CPR (e-CPR) is a community education program that teaches people how to support another person experiencing an emotional crisis. It is a hope-based public health approach to building strong, resilient, cooperative communities. There will be an in-person training session in Sheboygan on April 22 and April 23 from 9:00am to 4:00pm.
